Meaning of Early Postgastrectomy Gastritis in Remnant Stomach


Abstract
For the purpose of finding the clinical meaning of early postgastrectomy gastritis, prospe-ctive study was made by comparing between endoscopic finding of remnant stomach during 11 months period starting from Sept. of 1983 to July of 1984 at Guro Hospital, Department of General Surgery, College of Medicine, Korea University.
There ware 65 cases(95.6%) at 2 week and 51 cases(75. 0%) at 4 week with gastritis among 68 patients who accepted endoscopic examinations in 107 gastrectomized patients.
At 2 week, there 51 patients with a symptoms out of 68 patients and among these 51 patients, there 49(96%) gastritis patients. There were 17 patients with symptoms and among these 17 patients, there 16(94.1%) gastritis patients.
At 4 week, there were 55 patients with a symptoms out of 68 patients and among these 55 patients, there were 41(74.5%) gastritis patients. There were 13 patients with symptoms and among these 13, patients, there were 10(76.9%) gastritis patients.
There were 65 patients(95.6%) with bile reflux at 2 week and 54 patients(79.4%) at 4 week, and 38 patients (52.9%) with mucosal coating of bile juice at 2 week and 20 patients(29.4%) at 4 week which suggested fact that bile juice may be one of the causes of gastritis postope-ratively.
On the bases of high prevalence of gastritis postoperatively, surgeon should be willing to treat nonspecific symptoms of postgastrectomy, such as epigastric pain, postprandial fullness, nausea, vomiting, fatigue and loss of appetites rather than expected to wait and see.
